I agree that my son/daughter will participate in Do It Right Baseball's Fall Camps at GO Indoor Facility (4130 N. Rockwell Chicago, IL 60618). I have agreed to participate under my own free will and understand that Do It Right Baseball and GO Indoor Facility assume no risk for injuries that might be sustained during the camp. Participation is at the player's own risk, with the knowledge that baseball is a game with inherent risks of injury. The above named player and their guardian acknowledge this warning and consent to participate at their own risk.
